The Care Facilitator Supervisor, functioning under the supervision and guidance of the Center Operations Director and/or Clinical Dyad leader, is responsible for organizing and directing the work of the front desk staff, coordinating staff activities and schedules to ensure effective patient care services are provided, and ensuring quality standards are met. They train, guide, and support Care Facilitators to meet organizational front desk standards and provide them with the necessary tools and resources. The supervisor leads front office operations, including quality, compliance, human resources, patient experience, and direct supervision of employees. They also lead the VIP customer experience from entry to exit, ensuring patients and their families have a positive and memorable visit. Establishing and maintaining strong professional relationships with internal and external partners, they adhere strictly to established guidelines to provide the best solutions and options for patients, supporting the overall center experience.

ESSENTIAL JOB DUTIES / RESPONSIBILITIES :

  1. Lead a team of Care Facilitators and front desk staff, including setting performance goals, developing talent, and implementing improvements.

Engagement and Development :

  • Instill ChenMed values and behaviors
  • Build a strong culture and engagement
  • Promote team member retention
  • Provide clear onboarding expectations
  • Perform regular coaching and leadership rounding to support team development

Operational Excellence :

  • Follow the Center Playbook procedures and execute the core model
  • Understand and promote patient flow metrics and efficiencies
  • Focus on patient retention and positive customer experience, remaining accessible to staff and patients

Scheduling Optimization :

  • Adhere to scheduling templates, ensuring 100% patient scheduling
  • Schedule top 40 and risk score 70+ patients bi-weekly
  • Follow-up on IP/ER discharges and coordinate care scheduling

Partner with the growth team to ensure scheduling completion, ensure resources and priorities are clear for staff, participate in hiring, monitor standards, resolve escalated issues, support PCP scheduling, review notifications and documents, troubleshoot technical issues, and maintain supplies.

Other responsibilities include:

  • Cover for Care Facilitators as needed
  • Support new patient paperwork and tours
  • Perform other duties as assigned

KNOWLEDGE, SKILLS AND ABILITIES :

  • Strong business acumen and operational knowledge
  • Understanding of regulatory standards
  • Excellent decision-making and problem-solving skills
  • Strong organizational and leadership skills
  • Knowledge of medical insurance and EMR systems
  • Proficiency in Microsoft Office and IT systems
  • Effective communication skills in English

EDUCATION AND EXPERIENCE CRITERIA :

  • High school diploma or GED required
  • Leadership and coaching experience
  • Some college coursework preferred
  • Minimum 3 years' experience in a medical facility
  • BLS certification desired
