Job summary

Community House Mental Health Agency is seeking an FPATH Engagement Specialist in Seattle to provide comprehensive community supports for adults with multiple competency evaluations. You will work within a team, engage potential clients in jail and community settings, coordinate resources, and maintain HIPAA-compliant documentation.

The role is full-time (35 hours/week) with a schedule Monday–Friday, 9:00 am–4:00 pm, and requires a social services degree, driving access, and licensure

Qualifications

• Bachelor’s or Master’s degree in social services related field
• Legible handwriting
• Valid driver’s license with clean driving record for agency vehicles
• Experience with target population or willingness to receive targeted training
• Basic understanding of the criminal justice system
• Sensitivity to multi-cultural populations and issues
• Ability to attain Washington State Agency Affiliated Counselor License and H-Badge
• Ability to perform duties with reasonable accommodation

Responsibilities

• Visit King County Jails to engage potential FPATH clients and answer questions
• Collaborate with King County Forensic Navigators & Diversion Navigators
• Complete intake documentation as needed
• Serve as the face of FPATH to potential clients
• Outreach to encampments and community locations
• Coordinate client needs and appointments with community supports
• Transport clients to intake appointments when needed
• Attend team meetings and maintain professional collaboration with partners
• Ensure timely documentation and HIPAA confidentiality
