Community House Mental Health Agency in Seattle seeks an FPATH Engagement Specialist to provide comprehensive community supports for adults with recurrent competency evaluations. You will work as part of the FPATH team to connect clients to behavioral health treatment and linkages, using harm-reduction, trauma-informed, client-centered approaches.

Daily schedule is Mon-Fri 9:00 am-4:00 pm, full-time; you will engage in offices, jails, hospitals, encampments, and other community sites,

Responsabilités

• Go into the King County Jails to engage potential new FPATH clients in the program; answer any questions and ideally get “buy-in” for the program
• Work with King County Forensic Navigators and Diversion Navigators in and out of the jail system
• Complete intake documentation as needed
• Serve as the face of FPATH to potential clients
• Outreach to encampments, street-based, or other community-based locations
• In-reach to jails, hospitals, participant housing, and/or other facilities where participants may frequent
• Locate, build rapport, and enroll new FPATH referrals
• Locate and re-engage enrolled FPATH participants who are out of contact with the FPATH team
• Tracking active and inactive referrals and coordinating with FPATH teams
• Provide immediate crisis intervention and resolution as needed
• Coordinate client needs such as appointments, engagement with community support networks, vocational services, or other applicable needs
• Must be able to work effectively in a team environment and the ability to collaborate effectively with external partners including Forensic Navigators and Diversion Navigators and other legal entities
• Understanding the importance of clear, professional boundaries
• Assist with transporting clients to intake appointments
• Attend all team/staff/community/agency meetings and interact in a collaborative, professional, and constructive manner
• Timely completion of required documentation to program standards
• Adhere to HIPAA/Confidentiality standards
• Other duties as assigned
