Job summary

Community House Mental Health Agency in Seattle, WA is seeking an Engagement Specialist for the FPATH team to provide comprehensive community supports for adults with two or more competency evaluations within 24 months. You will engage clients in jail and community settings, coordinate intake, and link to behavioral health resources.

Qualifications

• Bachelor’s or Master’s degree in social services related field is required or preferred.
• Eligibility to obtain WA State Agency Affiliated Counselor License is required.
• Must be eligible for H-Badge from King County and able to access jail facilities.

Responsibilities

• Go into King County Jails to engage potential FPATH clients and obtain program buy-in.
• Work with Forensic Navigators & Diversion Navigators in and out of the jail system.
• Complete intake documentation as needed.
• Be the face of FPATH to potential clients.
• Outreach to encampments, street-based or community locations.
• In-reach to jails, hospitals, participant housing and other facilities.
• Locate, build rapport, and enroll new referrals.
• Re-engage enrolled participants who are out of contact.
• Track active/inactive referrals and coordinate with FPATH teams.
• Provide crisis intervention and resolution as needed.
• Coordinate appointments and engagement with community supports and services.
• Collaborate effectively with external partners and maintain boundaries.
• Assist with transporting clients to intake appointments.
• Attend team/staff/community/agency meetings; complete documentation timely.
• Adhere to HIPAA/Confidentiality standards.
