Job summary

Medix is seeking a Care Navigator to own the care experience for a growing panel of senior patients and their families in New York. You will develop thoughtful, individualized care plans and coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other healthcare partners to ensure seamless follow-ups and ongoing needs.

Qualifications

• Must hold an active RN/NP/PA license.
• Ability to work independently and own a patient population with minimal day-to-day direction.
• Strong clinical judgement; assess complex cases and develop individualized care plans.
• Startup mindset; comfortable in evolving processes, workflows, and best practices.

Responsibilities

• Own the care experience for a growing panel of senior patients and their families.
• Develop individualized care plans based on patient health, goals, and circumstances.
• Coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other partners.
• Manage post-hospitalization follow-ups and ongoing care needs.
• Coordinate referrals, appointments, and other services.
• Support medication reconciliation and identify gaps across providers and pharmacies.
• Help patients navigate insurance denials, prior authorizations, and appeals.
• Maintain consistent communication with patients and families.
• Attend or support appointments when appropriate.
• Ensure follow-up items and action plans are completed.
• Serve as a trusted resource for patients and families.
