Job summary

Medix is seeking a Care Navigator to own the care experience for a growing panel of senior patients and their families in New York. You will develop thoughtful, individualized care plans and coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other healthcare partners to ensure seamless follow-ups and ongoing needs.

Responsibilities

  • Own the care experience for a growing panel of senior patients and their families.
  • Develop individualized care plans based on patient health, goals, and circumstances.
  • Coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other partners.
  • Manage post-hospitalization follow-ups and ongoing care needs.
  • Coordinate referrals, appointments, and other services.
  • Support medication reconciliation and identify gaps across providers and pharmacies.
  • Help patients navigate insurance denials, prior authorizations, and appeals.
  • Maintain consistent communication with patients and families.
  • Attend or support appointments when appropriate.
  • Ensure follow-up items and action plans are completed.
  • Serve as a trusted resource for patients and families.

Job description


  • Own the care experience for a growing panel of senior patients and their families.

  • Develop thoughtful, individualized care plans based on each patient's health, goals, and circumstances.

  • Coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other healthcare partners.

  • Manage post-hospitalization follow-ups and ongoing care needs.

  • Coordinate referrals, appointments, and other necessary services.

  • Support medication reconciliation and identify potential gaps or issues across providers and pharmacies.

  • Help patients and families navigate insurance issues, including denials, prior authorizations, and appeals.

  • Maintain consistent communication with patients and family members.

  • Attend or support appointments when appropriate.

  • Ensure important follow-up items and action plans are completed.

  • Serve as a consistent and trusted resource for patients and their families.


Build the Care Model


  • Independently manage an initial patient population while maintaining a high standard of care.

  • Identify recurring challenges and opportunities within the patient experience.

  • Help develop repeatable workflows, processes, scripts, escalation protocols, and handoffs.

  • Document best practices and lessons learned from working directly with patients.

  • Partner with technology and product stakeholders to identify ways to improve care delivery.

  • Provide a clinical perspective on the development of tools, workflows, and technology.

  • Help establish the foundation for a scalable care-delivery model.


Build & Lead the Future Care Team


  • Help define the skills, backgrounds, and characteristics needed for future clinical team members.

  • Participate in recruiting and onboarding future Care Navigators.

  • Train and mentor new team members using the processes and best practices you help establish.

  • Help establish quality standards for patient care.

  • Eventually take responsibility for building, leading, and overseeing a growing care team.

  • Help ensure patients receive a consistent, high-quality experience as the organization scales.

  • Serve as a clinical leader and advocate for the patient as the organization grows.


Required Skills:


  • Active RN/NP/PA license

  • Demonstrated ability to work independently and take full ownership of a patient population with minimal day-to-day direction.

  • Strong clinical judgment and the ability to assess complex situations, prioritize needs, and develop thoughtful, individualized care plans.

  • Startup mindset: comfortable operating in an environment where processes, workflows, and best practices are still being built.

  • Growth mindset: eager to learn, seek feedback, experiment, identify opportunities for improvement, and continuously raise the standard of care.

  • Demonstrated potential to grow into a leadership role, including hiring, mentoring, developing, and eventually overseeing a team of Care Navigators.

  • Comfortable collaborating cross-functionally with clinical, operational, product, and technology teams.

  • Willingness to work five days per week onsite.
