Job Description:

About the Job:

Assist providers as needed, including rooming patients, taking vitals, triaging, calling in prescriptions, and other related responsibilities.

What You’ll Do:

• Perform nursing duties via in-person visits, telephone triage, and telehealth modalities.
• Assess patient needs, plan care, intervene, and evaluate progress.
• Collaborate with the interdisciplinary team to design and implement care plans.
• Administer medications, treatments, vaccines, perform point-of-care testing, and handle specimens within legal scope.
• Educate patients on self-management, medications, nutrition, and wellness activities based on collaborative care plans.

Who You Are:

• Current NH RN license required.
• Current BLS/CPR certification required.
• Previous healthcare experience required.
• Epic experience is a plus.
