Qualifications

  • Minimum of two years’ recent acute care experience.
  • Current RN or LPN license with New Hampshire’s Board of Nursing.

Responsibilities

  • Conduct in-person meetings with patients and families as needed.
  • Participate in community collaborations with hospital partners.

Description

Responsibilities include in-person meetings with patients and families as needed, participation in community collaborations with hospital partners, and documentation in electronic medical records (preferably EPIC). The role allows for a hybrid schedule, with a focus on excellent customer service, knowledge of hospice and home care, and strong communication skills. This is a part-time position, weekends only.

Qualifications
  • Graduate of an accredited nursing school (LPN or RN)
  • Nursing diploma or associate degree in nursing
  • Current RN or LPN license with New Hampshire’s Board of Nursing
  • Minimum of two years’ recent acute care experience
  • Ability to exercise initiative and independent judgment
Preferred Qualifications
  • Bachelor’s degree in nursing
  • Community health or education experience
  • EMR software proficiency
Required
  • Satisfactory criminal record clearance
  • Satisfactory motor vehicle record
  • Bureau of Elderly and Adult Services (BEAS) clearance
