Responsibilities

• Facilitates home studies and licensing needs on assigned caseload in SACWIS/Extended Reach.
• Completes visits and timelines for licensing; initiates within 1 week of assignment.
• emails and shares licensed family information; moves to open bed list.
• Coordinates fast-track interviews and training completion; licenses within 30 days of training.
• Attends Encourage Team meetings and participates in quality improvement activities.
