Job summary

National Youth Advocate Program is seeking a dedicated advocate to support children and families in Homewood, IL. The role focuses on case management, direct services, and collaboration with foster caregivers and partners to promote safety and successful community living.

You will mentor colleagues, document care accurately, and drive outcomes within regulatory and program requirements, contributing to NYAP’s mission of empowering youth and families.

Qualifications

• Bachelor’s degree in Social Work or comparable Human Services field; Master’s degree preferred.
• Minimum 2 years of direct service experience with youth and families.
• Willingness to work flexible/non-traditional hours.
• Ability to work with diverse cultures and communities.

Responsibilities

• Actively supports, represents, and extends the mission, vision, and values of the organization.
• Provides case management and other professional services, meeting or exceeding established outcomes thresholds on behalf of persons served.
• Provides direct services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
• Completes all required documentation in a timely manner.
• Completes all required training in a timely manner.
• Regularly reviews and modifies youth based plans to assure consistent progress and success on written plan goals.
• Establishes and maintains strong relationship with assigned foster parents and families of origin.
• Assists in family reunification and/or stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan.
• Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served.
• Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.
