Job summary

National Youth Advocate Program is seeking a Family Service Coordinator in Indiana to advocate for youth and families, providing in-home services, transportation, parenting education, counseling, and resource referrals, while maintaining timely documentation and coordinating with partners. Daily travel, evenings and weekends may be required to meet outcomes.

Qualifications

• Bachelor's Degree in Social Work or comparable Human Services field required.
• 2 years of direct service work with youth and families preferred.
• Flexible availability and willingness to work non-traditional hours.
• Valid driver’s license with reliable transportation and adequate auto insurance.
• Ability to work with diverse populations with respect and professionalism.

Responsibilities

• Actively supports, represents, and extends the mission of the organization.
• Provides in-home supportive services, transportation, parenting education, counseling, and referrals.
• Travels daily to deliver community-based services.
• Completes required documentation in a timely manner.
• Coordinates and monitors services with youth and family in the community.
• Reviews and updates youth-based plans to show progress toward goals.
• Builds strong relationships with foster parents and families of origin.
• Supports reunification or stability efforts for youth’s primary family.
• Provides safety assessment and crisis response when needed.
• Participates in Continuous Quality Improvement activities and weekly supervision.

Education
