Job summary

Good Shepherd Services in Bronx, NY is seeking a dynamic Case Planner to support timely permanency planning for children and youth in foster care. This in-office role includes field visits and a hybrid schedule.

Responsibilities include assessing safety, coordinating visits with families, preparing reports, attending hearings, and collaborating with a multidisciplinary team. Bachelor's degree and MSW or related experience preferred; bilingual English/Spanish encouraged.

Qualifications

• Bachelor’s Degree in related field required.
• MSW or at least one year related experience preferred.
• Must be able to establish working relationships with clients in need of counseling and supportive services.
• Must be able to effectively engage parents.
• Ability to apply theory of change.
• Willingness to facilitate groups with parents and foster parents.
• Ability to work cooperatively with supervisor(s), KEEP, PTC, and YDSC consultants, ACS and other team members.
• Excellent written and verbal skills necessary.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess and monitor for safety and risk.
• Establish immediate contact with families and foster families.
• Conduct casework contacts with child, parent, foster parent and collateral contacts; make referrals as needed.
• Visit family and child to gather social history and assess needs.
• Coordinate and supervise visits between family and child and sibling visits.
• Share responsibilities with team for permanency planning.
• Locate resources and referrals for family and child.
• Evaluate child’s adjustment and monitor school performance.
• Monitor foster parents and provide guidance for future care.
• Keep case records up to date and complete ACS/Agency documentation.
• Attend court appearances and permanency hearings.
• Participate in Family Team Conferences and related meetings.
