Job summary

SCO Family of Services in the New York area is seeking a Case Planner to support safety, permanency, and well-being for children and youth in foster care. You will work with families to develop and monitor service plans, assess risks, coordinate referrals (Health Homes, CFTSS, preventive services), and document progress.

The role requires a bachelor’s degree (BSW/MSW preferred), strong oral and written communication, and proficiency with Microsoft Office, Excel, and the Connections system.

Qualifications

• Bachelor’s degree required; BSW/MSW preferred.
• Excellent oral and written communication.
• Proficiency with Microsoft Office, Excel, and Connections.

Responsibilities

• Complete initial face-to-face contact with the child within 2 business days.
• Conduct ongoing face-to-face contacts to assess safety and risk.
• Complete safety checklists and risk questionnaires monthly.
• Count and monitor medication and medication compliance.
• Document progress in Connections within 7 business days.
