Qualifications

• Must possess a master’s degree in social work or a related field with a field placement in casework services to children and families or one year of experience; or a bachelor’s degree in social work or related field with one year of experience; or a bachelor’s degree in any field with two years of casework experience.

Responsibilities

• Develops and monitors child-specific treatment plans and interventions.
• Facilitates quarterly treatment team meetings and coordinates signatures.
• Coordinates visitation with biological family to promote positive change and preserve connections.
• Provides case management to link youth with medical, educational and social services.
• Monitors placement to ensure basic needs and permanency goals are met.
• Attends and participates in IEP, court proceedings, and related meetings.
• Maintains case records and documentation per standards.
