Job summary

HopeTree Family Services seeks a Foster Care Case Worker in Salem, VA. This full-time role provides case management for youth in foster care and treatment foster care, coordinating services and supporting provider families.

The candidate will address social, emotional, and developmental needs and participate in team meetings and planning. The ideal candidate demonstrates emotional maturity, strong judgment, and leadership.

Qualifications

• Must possess either a master's degree in social work or related field with a student placement in casework services to children and families or one year of related experience.
• Bachelor's degree in social work or related field with one year of casework experience for children and families.
• Bachelor's degree in any field with two years of casework experience for children and families.

Responsibilities

• Develops and monitors child-specific treatment plans and implements interventions to support goals.
• Facilitates quarterly treatment team meetings and ensures participation and proper documentation.
• Coordinates visitation with biological family to promote positive change and preserve connections.
• Provides case management, links youth to medical, educational, and social services.
• Monitors placement to ensure basic needs and development are met.
• Attends FPM/TDM, FAPT, IEP, and court meetings; represents the agency professionally.
• Maintains therapeutic relationships with youth, provider families, and birth families; upholds boundaries.
