Job summary

HopeTree Family Services is seeking a Case Worker Trainee to provide case management for youth in foster care and treatment foster care. The role involves coordinating services, monitoring treatment plans, and working closely with provider families to meet medical, educational and psychosocial needs.

The ideal candidate will have advanced education in social work or related fields and experience with children and families, strong judgment, and collaboration skills.

Qualifications

• Master's degree in social work or related field with supervised placement or experience in child and family casework.
• Bachelor's degree in social work or related field with 1 year of casework experience.
• Bachelor's degree in any field with 2 years of casework experience with children and families.

Responsibilities

• Develops and monitors child-specific treatment plans and implements interventions.
• Facilitates quarterly treatment team meetings and ensures participation/signatures.
• Coordinates visitation with biological family to preserve connections.
• Provides case management to minimize fragmentation and link youth to services.
• Monitors placement to meet basic, developmental, psychosocial and educational needs.
• Participates in FPM/TDM, FAPT, IEP, VEMAT, BID, and court proceedings.
