Job summary

Georgia MENTOR, a member of the Sevita family, is seeking a Case Manager for Therapeutic Foster Care in Savannah, GA. This role focuses on coordinating services, case management, and treatment plans to support individuals served and their families.

Qualified candidates have a Bachelor's degree in human services or related field, with preferred 1 year of relevant experience. A valid driver's license and state-specific licensure training are required.

Qualifications

• Bachelor's degree in human services or related field.
• One year of work-related experience with individuals with intellectual and developmental disabilities, brain injury, youth at-risk, etc. preferred.
• Equivalent combination of education and experience considered.
• Current driver's license, car registration, and auto insurance.

Responsibilities

• Coordinate development, review, and implementation of services, case management, behavior intervention plans, and treatments for individuals served.
• Oversee all services provided to individuals and families, monitor quality of care, ensure compliance with contractual terms and regulatory program requirements.
• Assist in developing quality services and engaging activities to meet individual needs.
• Build and maintain relationships with families and external case managers.

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
• Assist in the development of quality services and engaging activities that meet the individual served needs.
• Build and maintain relationships with families and external case managers.

• An equivalent combination of education and experience.

• Licensure and training as required by state such as CPR, First Aid, Behavioral Intervention Techniques, etc.
