Job summary

New Jersey Mentor, part of the Sevita family, is seeking a Case Manager to support youth in care across Burlington and nearby counties. The role is based in Mt. Laurel with responsibilities that include coordinating care, home visits, and maintaining compliant records.

The ideal candidate has a Bachelor's in human services, at least three years in case management, and strong verbal/written communication. Excel skills and independence are valued.

Qualifications

• Bachelor’s degree in human services.
• Three years of related work experience and knowledge of case management.
• Valid driver’s license, registration, and insurance.
• Ability to establish working relationships with individuals served.
• Demonstrated competence in verbal and written communication skills.
• Ability to handle crisis situations.
• Working knowledge of Excel
• Ability to learn other relevant computer systems
• Ability to work independently

Responsibilities

• Utilize Windows programs to track data for multiple rolling deadlines related to youth in care.
• Communicate with external team partners including DCPP, CMO, and family members of youth.
• Conduct home visits and office-based services, and accompany individuals being served as needed.
• Responsible for the annual contractual review process for Host Home Providers.
• Maintain and audit files regularly for compliance.
• Coordinate care with team members to ensure youth receive high-quality care.
• Exceptional organization and time management practices required.
• Flexible in daily schedule to best facilitate program needs.
• Interface effectively with internal team members to ensure licensing regulations are met.
• Performs other related duties and activities as required

$45,000.
