Caritas Family Solutions in Illinois is seeking a Foster Care Case Manager to collaborate with DCFS, youth, birth families, foster parents, and other supports to facilitate permanency for youth in care. The role requires visiting child and family teams, identifying barriers to permanency, and helping secure needed supports and services.

The position is full-time and exempt, reporting to the Foster Care Supervisor, with a starting compensation of $58,000 annually.

Qualifications

• Requires a Bachelor's Degree in Social Work or related Human Services field.
• Two years of experience required for specialized casework assignments.
• Strong written and verbal communication skills and organizational abilities.
• Ability to maintain accurate case records and comply with agency, DCFS, and court documentation requirements.

Responsibilities

• Provides case management services to children and families in foster care.
• Maintains and monitors youth in care placements for safety, permanency and well-being.
• Maintains current and accurate documentation of contact notes, case notes, and client records.
• Coordinates and links to agency and community resources to achieve permanency.
• Establishes a therapeutic alliance and engages families, caregivers, and service providers.
• Maintains contact with therapists, attorneys, CASA, and other providers; participates in Child and Family Team meetings.
• Attends and testifies in court hearings as necessary.
