Qualifications

• Bachelor’s degree in social work, drug and alcohol education, counseling, psychology, education, sociology, or criminal justice.
• Minimum of one year of professional employment in protective social services, family preservation or targeted case management.
• Ability to work in a team and mobilize community resources.

Responsibilities

• Make timely decisions on client services for medical, education, social, and emotional supports.
• Ensure 24/7 emergency availability for child, family, and state agency.
• Formulate permanency goals and plans for assigned clients.
• Prepare records and documentation of all work with clients.
• Facilitate meetings with biological parents, child, and caregiver in home settings.
• Lead Family Support Team and Team Decision Making meetings.
• Coordinate parent/child/sibling visits as required.
• Identify and link community resources for clients.
• Document outcomes, collect data, and report progress of services.
• Coordinate with Epworth staff, state agencies, courts, and providers; attend court hearings.
• Transport clients to appointments, court, school, and moves as needed.
• Secure legal permanency for each case; participate in peer record reviews.
