Arrow Child & Family Ministries is seeking a Case Manager to coordinate treatment and services for foster/adopt children and their families. You will manage intake, develop assessments and treatment plans, and monitor compliance with licensing standards while maintaining thorough documentation.

The role involves direct collaboration with foster homes, biological families, and schools; you'll provide consultation, crisis intervention, and education liaison duties as part of a multidisciplinary

Formación

• Bachelor's Degree in Social Work, Social Services, or related field with at least 1 year of experience.
• Experience working with children and families.
• Master's Degree preferred.

Responsabilidades

• Facilitate placement of children in foster and adoptive homes and ensure intake paperwork is signed and entered into the electronic case management system within Arrow policy timelines.
• Participate in development of assessment, treatment planning, service delivery, discharge, and aftercare as determined by the child’s service package.
• Assist and monitor families in complying with licensing/regulatory standards to maintain service package requirements and credentialing.
• Implement Pre-Admission Assessment and Individual Treatment Plans with foster parents, clients, and treatment team.
• Provide consultation for foster parents, adoptive parents, biological parents, respite provider, childcare staff, and other caregivers.
• Maintain accurate, timely case notes, billing documentation, treatment plans, progress notes and other documentation according to standards.
• Attend trainings and meetings as required.
• Serve as the child’s education liaison with the referral agency and foster parents.
• Coordinate case planning conferences, Professional Consulting Team, school meetings, court hearings, and staffing for cross-system collaboration.
• Maintain regular contact with family and child, monitoring well-being through phone, email, and home visits.
• Help families develop a therapeutic milieu consistent with the agency’s treatment model.
• Provide crisis intervention as needed to prevent escalation of high-risk behavior, relapse, or placement disruption.
• Complete discharge summaries and provide after-care services as required by the youth’s service package.
• Maintain child/family records in Arrow’s electronic client information system.
