Children’s Hope Alliance is seeking a dedicated Foster Care Case Manager to support children, empower foster families, and help build stable, nurturing environments where youth can thrive. This relationship-driven role demands advocacy, collaboration, and a daily commitment to making a meaningful impact.

You will coordinate services, lead Child and Family Team Meetings, coach foster parents, conduct in-home visits, and ensure documentation and licensing standards are met, while collaborating

Qualifications

• Bachelor's degree in Psychology, Social Work, or a related field.
• At least 2 years of experience working with children, families, or in a related human services role.
• Qualified Professional (QP) designation is required.
• Ability to respond to urgent situations and participate in on-call rotation.

Responsibilities

What You’ll Do

Champion Care for Youth & Families

• Serve as a trusted partner to children, foster families, and care teams—guiding them throughout the foster care journey
• Coordinate services, treatment planning, and resources to support each child’s unique needs
• Facilitate Child and Family Team Meetings and actively contribute to admission, treatment, and discharge planning

Support & Empower Foster Families

• Provide ongoing coaching, training, and hands-on support to foster parents
• Conduct regular in-home visits to assess strengths, address needs, and ensure a safe, nurturing environment
• Help families navigate behavioral challenges using proven models like the Teaching Family Model

Be a Consistent Advocate

• Maintain strong communication with biological families, social workers, and community partners
• Represent Children’s Hope Alliance in court hearings, treatment reviews, and external meetings
• Respond to urgent needs and provide crisis/on-call support when it matters most

Ensure Quality & Compliance

• Keep accurate, timely documentation and case records using clinical management systems
• Monitor foster home compliance with licensing standards and conduct ongoing reviews
• Review medical administration records to ensure the safety and well-being of children
