Job summary

Children’s Hope Alliance in Morganton, NC is seeking a Foster Care Case Manager to support children, empower foster families, and build stable, nurturing environments. You will coordinate services, lead team meetings, provide in-home support, and advocate for families while ensuring documentation and licensing compliance.

A Bachelor's in psychology or social work and a QP designation are required, with at least two years of relevant experience.

  • Bachelor’s degree in Psychology, Social Work, or a related field.
  • Qualified Professional (QP) designation is required.
  • Ability to respond to urgent situations and participate in on-call rotation.
  • At least 2 years of experience with children or families.

Responsibilities

  • Serve as a trusted partner to children, foster families, and care teams throughout the foster care journey.
  • Coordinate services, treatment planning, and resources for each child.
  • Facilitate Child and Family Team Meetings and contribute to admission, treatment, and discharge planning.
  • Maintain accurate case records and ensure compliance with licensing standards.

What You’ll Do
  • Serve as a trusted partner to children, foster families, and care teams—guiding them throughout the foster care journey
  • Coordinate services, treatment planning, and resources to support each child’s unique needs
  • Facilitate Child and Family Team Meetings and actively contribute to admission, treatment, and discharge planning
Support & Empower Foster Families
  • Provide ongoing coaching, training, and hands-on support to foster parents
  • Conduct regular in-home visits to assess strengths, address needs, and ensure a safe, nurturing environment
  • Help families navigate behavioral challenges using proven models like the Teaching Family Model
Be a Consistent Advocate
  • Maintain strong communication with biological families, social workers, and community partners
  • Represent Children’s Hope Alliance in court hearings, treatment reviews, and external meetings
  • Respond to urgent needs and provide crisis/on-call support when it matters most
Ensure Quality & Compliance
  • Keep accurate, timely documentation and case records using clinical management systems
  • Monitor foster home compliance with licensing standards and conduct ongoing reviews
  • Review medical administration records to ensure the safety and well-being of children

What You’ll Need
