Job summary

Qualifications

Responsibilities

• Conduct telephonic and face-to-face member assessments.
• Coordinate individualized service plans and follow-up activities.
• Collaborate with members, families, caregivers, physicians, and interdisciplinary teams.
• Identify members who may benefit from additional services and support.
• Coordinate with clinical teams regarding potential healthcare needs.
• Maintain accurate documentation and travel to assigned locations as required.
