Qualifications

• Experience with Severe Mental Illness (SMI) and crisis intervention.

Responsibilities

• Conduct psychosocial assessments and develop individualized care and treatment plans.
• Participate in interdisciplinary team meetings to support client goals and progress.
• Provide case management, counseling, and rehabilitative skills to patients.
• Coordinate services among clinical staff, social workers, and community providers.
• Monitor client participation and document progress and outcomes.
• Develop discharge plans and coordinate aftercare and housing referrals.
• Facilitate psychoeducational groups and crisis intervention as needed.
