CHD Center for Human Development in Springfield, MA is seeking candidates to provide direct outreach services to individuals living independently in the community with a forensic background.

Responsibilities include supporting daily living activities, administering medications, advocating for clients, promoting independence and recovery, and coordinating with criminal justice professionals to ensure compliance and effectiveness of care.

Qualifications

• Bachelor’s Degree in Social Work or related field or relevant experience.
• Strong written, verbal and interpersonal skills.
• Ability to work in team and independently.
• Background check clearance required.
• Vehicle and valid driver’s license required.
• Medication administration training or willing to complete.

Responsabilités

• Support persons served in activities of daily living.
• Administer medications as needed.
• Advocate for individuals in various community settings.
• Provide opportunities to optimize independence, rehabilitation and recovery.
• Develop professional relationships and provide person-centered counseling.
• Liaise with criminal justice professionals to support compliance.
