Job summary

Horizon House, Inc. is seeking qualified candidates to join its Delaware County FACT team, providing case management and direct clinical services for adults with behavioral health needs. Qualifications

• Bachelor's Degree in Human Services required.
• Certified Addiction Counselor (CAC) preferred, with ongoing coursework for re-certification.
• Strong communication and interpersonal skills essential.

Responsibilities

• Provide service coordination (case management) for assigned clients and coordinate activities of the treatment team.
• Develop, write, implement, evaluate and revise treatment goals and plans with clients and the ITT.
• Provide individual supportive therapy and psychotherapy to clients in office and community settings.
