Community Health Resources (CHR) is seeking a Forensic Nurse Specialist- LPN for DOC Cybulski in Enfield, CT. This full-time on-site role runs Mon-Fri, 4:45am-1pm, at DOC Cybulski, Enfield, CT.

Responsibilities include Medication Assisted Treatment for inmates, inputting into the EHR, and coordinating with the OTP team and DOC staff. LPN licensure is required for full-time, RN or LPN for per diem positions, with background checks completed.

Qualifications

• Nursing education for LPN; for Per Diem LPN or RN.
• Experience in OTP preferred.
• LPN for full-time; LPN or RN for Per Diem with background checks.

Responsabilités

• Provide Medication Assisted Treatment for inmates with substance use or co-occurring disorders.
• Input data into the electronic health record accurately.
• Prepare and dispense medication doses as ordered by a physician.
• Monitor patient responses and withdrawal symptoms.
• Coordinate with CHR OTP, DOC Safety, and Health and Addictions Services.
