Job summary

Community Health Resources in Enfield, CT is seeking a Forensic Nurse Specialist - LPN for the Department of Correction program. This full-time role supports MAT for inmates and works Monday through Friday, 4:45am to 1:00pm.

The LPN will prepare doses, administer MAT medications, document in the electronic health record, monitor patients, and coordinate with CHR OTP team and DOC staff. Prior OTP experience is preferred but not required.

Qualifications

• Licensed practical nurse (LPN) required; RN allowed for Per Diem positions.
• Experience in opioid treatment programs preferred but not required.
• Good standing licensure and ability to pass background checks.

Responsibilities

• Perform ongoing evaluation of treatment response and vital signs.
• Prepare and dispense medication doses as ordered.
• Administer FDA-approved MAT medications as directed.
• Input data into electronic health records accurately.
• Coordinate admissions and discharge documentation.
• Maintain compliance with regulatory standards and policies.
