Pathways Forward is seeking a Forensic Intake/Administrative Case Manager to provide assessment, case management, service coordination, advocacy, and support to individuals involved with the criminal justice system. The role collaborates with consumers, families, mental health providers, courts, and community agencies to promote continuity of care and access to services.

• Strong communication and organizational skills for forensic settings.
• Commitment to resiliency and recovery-oriented approaches in supporting individuals.
• Experience or familiarity with criminal justice and forensic mental health is a plus.

Responsibilities

• Conduct intake interviews and assessments to determine eligibility and needs for mental health services.
• Maintain a caseload within MH Jail Diversion Program and related systems.
• Coordinate services with courts, correctional facilities, providers, and community agencies.
• Attend court hearings and advocate for consumers as appropriate.
• Assess urgent situations and make referrals, including referrals for psychiatric admissions when needed.
• Coordinate access to mental health and case management services to ensure continuity of care.
• Maintain case records, documentation, and required reports.
