Job summary

Helio Health is seeking a Case Manager to support its mission of promoting recovery from substance use and mental health disorders. This full-time onsite role is located at the Onieda County Forensic Program with a Monday–Friday 7:30am–4:00pm schedule. In addition to a comprehensive benefits package, full-time hires are eligible for a Sign-On Incentive up to $1,500.

Pay is $23.00–$25.00 per hour. Responsibilities include intake, assessment, treatment planning, referrals, counseling, crisis

Qualifications

• Associate's degree in related field required; Bachelor's preferred.
• Qualified Health Professional (QHP) preferred.
• Two years of experience working with behavioral health and/or substance abuse issues.
• Valid NYS Driver's License preferred.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including intake, orientation, assessment, evaluation, and intervention (Only performed by Case Managers with QHP).
• Treatment planning, referrals, counseling, crisis intervention, patient/resident education, case management, reporting and record keeping.
• Coordination of care with other community providers and discharge planning.
• Trains staff, volunteers, and students; participates in QA activities and committees; represents agency to the community.

Overview

Helio Health is looking for a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This will be a Full-Time position located at our Onieda County Forensic Program. The schedule will be Monday-Friday, 7:30am-4:00pm.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including:
  • Screening, intake, orientation
  • Assessment, evaluation, and intervention (Only performed by Case Managers with QHP)
  • Treatment planning (Only performed by Case Managers with QHP)

  • Assists in problem resolution and implementation of supervisor approved program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents agency to the community.
• Other duties as assigned by supervisor.
