Job summary

Helio Health, Inc. in Rochester, NY is hiring a full-time Continuing Care Coordinator to provide rehabilitation services as part of a comprehensive treatment plan.

The role supports patients through ongoing care and coordination within the inpatient program. You will maintain a caseload, conduct counseling tasks such as screening, intake, assessment, treatment planning, referrals, crisis intervention, and documentation, and collaborate with staff and community partners.

Qualifications

• Associate's Degree in a human service field with CASAC-T required.
• One year counseling experience preferred.
• Equivalent combination of education and experience may be substituted.

Overview

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Continuing Care Coordinator will provide rehabilitation services to patients as a part of their overall treatment. The Continuing Care Coordinator will work on a Full-Time basis out of Helio Health’s Rochester Inpatient Program.

Pay Rate: $23.00 - $27.00 per hour.

In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!

Responsibilities

• Maintain a case load of patients seeking treatment from the effects of substance use and/or mental health disorders.
• Perform essential counseling responsibilities including screening, intake and orientation, assessment, evaluation and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting and record keeping.
• Participate in program, administrative and clinical committees as assigned.
• Relate effectively with staff, clients, and community while upholding confidentiality requirements.

Qualifications

• Associate's Degree in a human service field with minimum of CASAC-T required.
• One year counseling experience preferred.
• Equivalent alternative combination of education and experience may be substituted for the educational requirements.
• Knowledge of addiction/recovery, behavioral health, self-help group process and principles of counseling.
• Proficiency with basic computer applications.
