Job summary

Helio Health is seeking a Senior Case Manager to support recovery from substance use and mental health disorders. This onsite, full-time role focuses on coordinating services for patients in our Syracuse Supported Housing Program.

Responsibilities include caseload management, intake, assessment, treatment planning (for QHPs), referrals, counseling, crisis intervention, education, coordination of care, and discharge planning. Benefits include a sign-on incentive of up to $1,500.

Qualifications

• Associate's degree in related field required; Bachelor's degree preferred.
• Qualified Health Professional (QHP) preferred.
• Two years of experience working with behavioral health and/or substance abuse issues.
• Candidates with lived experience encouraged to apply.
• Valid NYS Driver's License preferred.

Overview

Helio Health is looking for a Senior Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full-Time position and located at our Syracuse Supported Housing Program.

In addition to Helio Health's comprehensive benefits package, this position is eligible for a Sign-On Incentive of up to $1,500!

Pay: $26.00 - $28.00 per hour.

Responsibilities

• Maintains a case load and performs primary case work responsibilities including:
  • Screening, intake, orientation
  • Assessment, evaluation, and intervention (Only performed by Case Managers with QHP)
  • Treatment planning (Only performed by Case Managers with QHP)
  • Referrals
  • Counseling
  • Crisis intervention
  • Patient/resident education
  • Case management

• Trains of staff, volunteers, and students/interns

  • Assists in problem resolution and implementation of supervisor approved program modifications.
