Job summary

Helio Health is seeking a Case Manager for its HUD Housing program in Syracuse, NY. This full-time, on-site role focuses on coordinating services for patients/residents in treatment programs and beyond, with a sign-on incentive of up to $1,500 and pay between $23.00 and $24.50 per hour.

The position requires a High School Diploma or GED (associate degree preferred), and at least two years of behavioral health or substance use experience. A valid NYS Driver’s License is required.

Qualifications

• High School Diploma or GED required; an associate degree is preferred.
• Two years of experience in behavioral health and/or substance use preferred.

Responsibilities

• Maintains a caseload and performs primary case work responsibilities: screening, intake, orientation; assessment and intervention; treatment planning; referrals; crisis intervention; patient education; case management; reporting and record keeping; consultation with other professionals; coordination of care with community providers; discharge planning.
• Trains staff, volunteers, and students/interns.
• Home visits required (agency vehicles may be provided).
• Performs quality assurance activities: follows policies, monitors services, evaluates programs, and supports program modifications.
• Participates in program administrative and clinical committees as assigned.
• Represents the agency to the community.
• Other duties as assigned by supervisor.

Overview

Helio Health is looking for a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full-Time position and located at our HUD Housing program.

In addition to Helio Health's comprehensive benefits package, this position is eligible for a Sign-On Incentive of up to $1,500!

Pay: $23.00 - $24.50 per hour.

Responsibilities
