Job summary

Apalachee Center, Inc. in Tallahassee is seeking a full-time Case Manager for Forensic Residential Out-Patient services.

You will coordinate residential and community care, communicate with the court, assess client needs, develop treatment plans, arrange referrals and transportation, and maintain thorough documentation to ensure continuity of care. Qualifications

• Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board or a Bachelor’s Degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation or related human services field.
• Must obtain CBHCM certification within 12 months if provisional status is earned.
• Must obtain ADM Targeted Case Manager Certification and maintain license.
• Must possess a valid driver’s license and Apalachee Driver status.

Responsibilities

• Provides case management services including assessment, planning, linking, monitoring and advocacy per agency and court policies and protocols.
• Completes Progress Notes and clinical documentation for all service activities per Client Record Handbook and agency policies.
• Meets productivity and CSP/SPR requirements as specified by policy and quality standards.
• Completes monthly progress reports to the probation court regarding client progress and compliance with the program.
• Tracks and coordinates transportation to court hearings and coordinates attendance when hearings fall outside the catchment area.
