Upstate Caring Partners seeks a FACT Team Leader to oversee a multidisciplinary team delivering community-based mental health services to justice-involved individuals with serious mental illness. You will lead daily operations, ensure resources, and collaborate with criminal justice and community partners to promote recovery and proper care.

The role emphasizes supervision of staff, adherence to regulations, and driving outcomes within budget and program standards.

Qualifications

• Leadership experience in a multi-disciplinary team setting.
• Experience serving justice-involved individuals is strongly preferred.
• Must have a valid NYS Driver’s License.

Responsibilities

• Oversee daily operations of the Forensic ACT team with fidelity to program guidelines.
• Provide direct supervision to a multidisciplinary team including clinicians and nurses.
• Ensure person-centered, recovery-oriented services addressing needs of individuals with SMI and justice involvement.
