Job summary

RWJBarnabas Health seeks a Program Coordinator to oversee the Food Farmacy project at Monmouth Medical Center. You will enroll participants, manage pantry distributions, and ensure grant deliverables are met, working with FulFill Food Bank and local farm partners to maintain inventory.

Qualifications

• Bachelor's degree in Nutrition and Dietetics or Public Health preferred.
• Proficient with Microsoft Office and Epic or other medical record systems.
• Bilingual abilities are a plus.

Responsibilities

• Register Food Farmacy patients referred by a healthcare provider and complete enrollment paperwork.
• Manage food pantry distribution hours and document visits.
• Conduct in-person or phone pre- and post-program surveys with patients.
• Receive and stock food pantry inventory from Fulfill and other partners.
• Collect enrollment and attendance data for monthly and annual reporting.
• Assist with partner billing and Food Farmacy administrative tasks.
• Collaborate with MMC's SNAP Navigator to ensure access to resources.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

The Food Access Coordinator is responsible for conducting day-to -day operations of the RWJBarnabas Health Food Farmacy project at Monmouth Medical Center. You will enroll participants, manage food pantry distributions and help to meet grant deliverable requirements for the program. You will work with our partners at FulFill FoodBank of Monmouth and Ocean Counties and additional farm partners to receive and maintain food pantry inventory.

As a Food Access Coordinator, a typical day might include the following:

• Registering Food Farmacy patients who are referred by their healthcare provider and completing all necessary enrollment paperwor k
• Managing food pantry distribution hours: packing client bags and documenting visits
• Conducting in-person or phone pre and post program intervention surveys with patients,
• Receiving and stocking food pantry inventory from Fulfill and other community partner organizations
• Collecting enrollment and attendance data for monthly departmental and annual community benefit reporting
• Assisting with community partner billing and Food Farmacy departmental administrative tasks as assigned,
• Collaborating with MMC s assigned SNAP Navigator to ensure comprehensive food access resources are available for all patients,
