• Assist individuals in identifying and communicating their requests and needs for services and support.
• Promote healthy lifestyle and wellness practices and prevent illness and accidents.
• Support self-advocacy and encourage independence in daily living.
• Accompany individuals in community activities to promote social engagement and inclusion.
• Develop independent living skills and encourage self-sufficiency.
• Provide safe transportation to appointments and community activities.
• Document services accurately to support communication among teams.
