Qualifications

• Two years of personal/professional experience working directly with adults with co-occurring diagnoses.
• Experience in crisis intervention, trauma-informed care and positive behavioral support.
• Valid PA/NJ Driver’s License with at least 1 year of driving experience.

Responsibilities

• Provides care and support to individuals following their ISP, ensuring safety and meeting living needs.
• Implements ISP outcomes and supports trauma-informed, recovery-oriented approaches.
• Documents services accurately and promptly, supporting health and wellness goals.
• Teaches life skills to enhance independence and community participation.
• Supports health, nutrition, exercise and sleep as stress management tools.
