You will provide community support to adults aged 18+ who have mild learning disabilities, helping with housing challenges, mental health issues, substance misuse and other complex needs.

Responsibilities

• Provide community support to adults 18+ with mild learning disabilities in community settings.
• Assist with housing stability, housing applications and accommodation processes; support service users to maintain suitable housing.
