Cleveland Clinic Main Campus is seeking a Care Management Assistant to help transition patients to post-acute care and coordinate with providers to fulfill discharge plans. You will work alongside a dedicated team to ensure smooth transitions and documentation.

As a CMA, you will support discharge processes, bed coordination, transport arrangements, and procurement of needed items, contributing to high-quality patient care in a leading health system.

Qualifications

  • High School Diploma or GED.
  • Three years of recent experience in healthcare or related field.
  • Fluency in medical terminology.
  • Computer literacy.

Responsibilities

  • Participate in discharge planning activities for complex patients.
  • Coordinate with Transitional Care Coordinators and Social Workers to ensure timely discharge and linkage with post-discharge providers.
  • Communicate discharge planning status of all patients referred by Transitional Care Coordinator.
  • Determine bed availability at extended care facilities and advise on openings.
  • Transmit initial patient demographic information as appropriate.
  • Coordinate paperwork for PASS-ID and other regulatory documents before transition.
  • Prepare information from the medical record before transition to post-acute settings.
  • Arrange transportation to post-acute care and document arrangements in the medical record.
  • Maintain ongoing communication with coordinators and social workers on transition plans.
  • Assist with procurement of medications, medical supplies, and other items for discharge.

As a Case Management Assistant, you will facilitate the transition of patients to post-acute care and assist with supply of procurement to fulfill discharge plans. You will communicate with extended care facilities regarding bed availability, ensuring necessary clinical documentation is supplied to providers before arranging transportation and documenting details of the transition. This position offers a unique way to make a positive difference in your community while enhancing your skills within an innovative healthcare system.

A caregiver in this position works days, 8:00am-4:30pm.

A caregiver who excels in this role will:

  • Participate in discharge planning activities for complex patients.

  • Partner with Transitional Care Coordinators and Social Workers to ensure a timely discharge and provide appropriate linkage with post-discharge providers.

  • Communicate with Transitional Care Coordinators and Social Workers regarding the discharge planning status of all patients referred by Transitional Care Coordinator.

  • Determine bed availability at extended care facilities based upon requests by patients and families and advise the Transitional Care Coordinator and Social Workers of bed opening.

  • Transmit initial patient demographic information as appropriate.

  • Coordinate paperwork for PASS-ID and all other regulatory documents required before transition of patients to post-acute care.

  • Prepare necessary information from the medical record before transition of patients to post-acute settings.

  • Arrange for transportation to post-acute care and document arrangements in the medical record.

  • Maintain ongoing communication with Transitional Care Coordinators and Social Workers of the status of transition plans assigned to CMA.

  • Assist with arranging for procurement of difficult to find medications, medical supplies and other items that will be needed to fulfill a patient’s discharge plan.

Minimum qualifications for the ideal future caregiver include:

  • High School Diploma or GED

  • Three years of recent experience working in healthcare or a related field

  • Fluency in medical terminology

  • Computer literacy
