Job summary

Trinity Health in Ann Arbor, MI seeks an ED Case Manager to coordinate admissions and ensure appropriate care timing. The role is full-time, 36 hours weekly, with a 12-hour shift pattern and rotating weekends/holidays, supporting the LV1 Trauma ER.

Qualifications

• Current Michigan Nursing License.
• Associate degree in Nursing; BSN required within 5 years of employment.
• Three years of recent case management, utilization review, or related experience.
• CMSA certification desirable: pursue certification within three years or in progress.

Responsibilities

• Coordinate actions for admitted or emergency center patients to receive the right care in the right place at the right time.
• Assess readmission-qualified patients and reasons for return; adapt plan accordingly.
• Document readmission info in the Epic CM system.
• Identify discharge barriers from the ED and develop strategies to address them.
• Collaborate with other disciplines to resolve discharge barriers and report findings to leadership.
• Consult with EC physician on admission status and level of care; ensure documentation meets hospital and insurance criteria.

Michigan Nursing License (RN)

Associate degree in Nursing
