Job summary

Brooks Skilled Nursing Facilities in Jacksonville, FL is seeking a Licensed Practical Nurse (Float) to perform nursing functions under the supervision of a registered nurse, follow care plans, and participate in care planning across Bartram Crossing and University Crossing locations.

The role requires Florida LPN licensure, at least 1 year of acute or post-acute inpatient experience, IV certification, and current CPR/BLS.

Qualifications

• Must be licensed as an LPN in the state of Florida.
• Must have 1 year of experience in acute care or post-acute care inpatient hospital.
• IV Certification required.
• Current hands-on CPR/BLS certification required.
• Must attend week of Training/Orientation, Monday–Thursday, 8:00 a.m.–5:00 p.m.

Responsibilities

• Documents observations and care given in accordance with company process and policies.
• Administers medications.
• Administers basic nursing care measures to rehab patients and assists with acutely ill rehab patients.
• Admits, transfers, and discharges patients.
• Observes, records, and reports subjective patient symptoms.
• Assists the RN in giving physical and emotional support to patients.
• Provides for dietary and fluid needs, including nourishment, feeding, IV fluids, and monitoring intake/output.
• Takes and records vital signs: temperature, pulse, respirations, and blood pressure.
