Job summary

Brooks Rehabilitation seeks a Licensed Practical Nurse (LPN) for Bartram Crossing in Jacksonville, FL, part-time night shift (10:45pm-7:15am). You will document, administer meds, and support rehab patients under RN supervision on the Bartram Crossing campus.

Qualifications

• Licensed as an LPN in the state of Florida.
• 1 year of experience in a skilled nursing facility is preferred.
• IV certification required and current hands-on BLS certification.

Responsibilities

• Documents observations and care in accordance with policies and procedures.
• Administers medications and basic nursing care to rehab patients.
• Admits, transfers and discharges patients as needed.
• Monitors vital signs and reports to RN with any changes.
• Provides support to patients and ensures dietary and fluid needs are met.
