Licensed Practical Nurse/LPN, Pool

Location: Bishops Woods, Waukesha County, 205 Bishops Way, Brookfield, Wisconsin, United States of America

Schedule: Multiple shifts across Milwaukee, Racine, and Waukesha counties, primarily 2nd and 3rd shifts, including weekends and holidays.

Job Summary: Community Care is hiring Licensed Practical Nurses (LPNs) committed to teamwork and providing excellent care. This pool position offers opportunities to work in residential, clinic, and home care settings, covering various shifts and sites.

Responsibilities:

1. Coordinate and provide care during weekday operations in collaboration with nursing staff.
2. Serve as care coordinator for weekend and holiday shifts.
3. Ensure care delivery complies with regulations, policies, procedures, and standards.
4. Travel to different sites and conduct home visits as needed.
5. Complete LPN competency checklists and onboarding requirements within specified timelines.
6. Attend mandatory training sessions.

Requirements:

• Diploma from an accredited practical nursing program.
• Current Wisconsin LPN license in good standing.
• Minimum of one year experience working with older adults or persons with disabilities in healthcare settings; practical nursing experience preferred.

Schedule Options:

• Tier 1: At least 2 shifts per month, including one weekend day and one major holiday annually.
• Tier 2: At least 4 shifts per month, including two weekend days and two holidays (one major, one minor).
