• Licensure with the Massachusetts Board of Registration in Pharmacy.
• ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended.
• Pharm.D. degree with minimum of 1 year of hospital experience

Responsibilities

• Review patient medication history, educate patients on drug therapy, establish outcomes and monitor therapy.
• Monitor pharmacokinetics and drug interactions; document interventions in the EHR.
• Counsel patients on discharge medications and assist with transition of care.
• Oversee pharmacy technicians and support accurate dispensing and regulatory compliance.
