UMass Memorial Health in Worcester, MA is seeking a clinical pharmacist to provide comprehensive medication management, patient education, and collaboration with the care team to improve safety and outcomes. You will review medication histories, monitor therapy, counsel patients, and contribute to discharge planning and ambulatory care efforts.

This role supports teaching, research, and interprofessional education.

Competenze

• Must have Pharm.D or BSPharm with 3 years hospital experience.
• Massachusetts pharmacist licensure required.
• ASHP residency (PGY1) preferred; hospital experience recommended.

Mansioni

• Provide medication management services in a collaborative manner with the care team.
• Review patient medication history and educate patients on therapy and outcomes.
• Monitor patient outcomes and adjust therapy as needed.
• Discharge counseling and education; support transitions of care.
• Supervise pharmacy technicians; support regulatory and quality initiatives.
