Job summary

Qualifications
- Doctor of Pharmacy (Pharm.D) or BSPharm with hospital experience.
- Massachusetts licensure to practice pharmacy.
- ASHP residency in Pharmacy Practice (PGY1) recommended.
- Hospital experience preferred for Pharm.D. degree.
Responsibilities
- Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication related issues in the EHR.
- Monitor pharmacokinetics and potential adverse drug interactions (drug–drug, drug–lab, drug–disease, drug–food).
- Counsel patients on discharge medications and provide drug information to health care professionals.
- Perform medication order verification and Discuss clarifications with prescribers; document interventions.
