Qualifications

• Requires a high school diploma or equivalent.
• Previous related experience in a healthcare setting preferred.
• Able to exercise effective judgment, sensitivity, and creativity to changing needs and situations.
• Ability to multi-task and perform multiple priorities.
• The ability to contribute in a team environment and/or independently, to provide excellent customer service.
• Ability to make decisions and solve problems.
• Demonstrates an attitude of cooperation and professionalism.
• Must be able to solve problems and critically think, sometimes during stressful situations.
• Listens well and effectively communicates in writing and orally.

Responsibilities

• Assist with planning and facilitating activities for individuals with disabilities and accompanying individuals on outings.
• Provide personal cares and documentation of goals, while maintaining confidentiality.
• Assist with meal preparation and feeding when required.
• Support mobility using walkers or wheelchairs as applicable.
• Record and report changes in individuals' conditions to supervisors.
• Provide transportation for community outings as required.
