Homes-For-Independent-Living in Wisconsin is hiring a driver to transport adult clients with disabilities to appointments, day programs, events and more. You will provide safe transportation in company vehicles and supervise individuals served during trips.

Qualifications

• Must have a valid WI driver's license in good standing to drive a company vehicle.

Responsibilities

• Provide safe transportation for individuals served to medical appointments, community activities, programs, errands, and other destinations.
• Supervise and support individuals served during transportation, appointments, and community activities while ensuring their safety and well-being.
• Support implementation of Individual Treatment Plans (ITPs), Behavior Treatment Plans (BTPs), and other care plans.
• Communicate appointment outcomes, provider updates, and relevant health information to appropriate team members.
• Maintain accurate and timely documentation including transportation logs, medical visit records, and incident reports.
• Assist with direct support services as needed, including supervision, coaching, and support with daily living or community integration activities.
• Maintain vehicles in safe operating condition, including inspections, cleanliness, and reporting maintenance needs.
